Scenario: A 35-year-old female presents to the clinic with complaints of asthma exacerbation. She was diagnosed with asthma as a child and has managed it with inhaled LABA and SABA. She reports that she started working in a bakery a few weeks ago, her asthma has worsened. She presents with allergic rhinitis symptoms which include wheezing, episodic cough, chest tightness, shortness of breath, itchy, red, and teary eyes, and a runny, itchy, stuffy nose. These symptoms worsen within 1 to 2 hours of entering her workspace and continue to worsen throughout the day. She notices an improvement within 1 to 2 hours of leaving the workplace. She wears a paper mask at work. She has had seasonal allergic rhinitis since she was a child. She smokes tobacco cigarettes one pack/ day for the last 12 years. She has a significant family history of asthma- mother and sister. She is currently using inhaled LABA daily and inhaled SABA as needed-up to 4 times per day for relief. Lab tests are conducted and the patient is diagnosed with baker’s asthma.
