NR536 Week 4: Collaboration Cafe Person-centered Strategies to Reduce Disparity

Imagine that you are a MSN-prepared nurse leader in an out-patient cardiovascular clinic that serves low-income and homeless individuals. Consider the needs of the patient population that you serve, and respond to the following:

  • As a result of cardiovascular disease, what specific health risks might be present within this vulnerable patient group?
  • What physical and social determinants of health might influence the cardiovascular health of patients within this population?
  • Describe three person-centered strategies that you suggest to promote the health of patients.
  • How will competency in advanced health assessment, pathophysiology, and pharmacology strengthen your ability to address the health risks experienced by this group of patients?

Solution: NR536 Week 4: Collaboration Cafe Person-centered Strategies to Reduce Disparity

The homeless and low-income people have various risks that increase their vulnerability to cardiovascular disease (CVD). These people due to poverty, are unable to afford quality care and thus, do not have equitable access to preventive and treatment care (Baggett et al., 2018). They cannot easily afford medications for CVD and thus, suffer higher morbidity and mortality rates. Again, low-income and homeless people, due to poor financial status, cannot easily afford the healthy diet and nutrition needed for health promotion and prevention of CVD (Palmer, 2020). Homeless and low-income people have poor living conditions e.g. tents exposed to cold or houses with poor ventilation. Also, due to poverty, these people are more likely to engage in substance use (e.g. alcohol, smoking, and illicit drug use), increasing their vulnerability to CVD.

The physical and social determinants of health that influence cardiovascular health for low-income and homeless people include low or lack of resources e.g. healthy foods, educational and employment opportunities, and living wages; social attitudes e.g. discrimination by the society, exposure to unhealthy physical environment e.g. trash and living in crowded areas, and exposure….Please click purchase button below to get full answer at $5
